Child's name is a __ year old female/male with a complex history and present diagnosis of Tree Nuts, Peanut.
Any contact with open cuts, eyes, mouth or even traces of the above mentioned could make him very sick and even lead to a potentially fatal allergic reaction requiring injectable Epinephrine (Epi Pen/Epi Pen Jr) plus Benadryl. An ambulance must be called to avert serious or fatal outcome.
Child's name requires specialized supervision all day for the entire year including on the school bus.
